We do require intake forms to be filled out prior to your initial visit in order to best address your needs. Lab work will mostly likely be ordered at this appointment, which will be an additional charge.

Your follow up visit is where we will review your initial lab work and formulate a plan of care. You will have the opportunity to establish a membership at this time.  If you chose to proceed with a Thrive Wellness Studio Membership, the cost of your initial follow up visit will be credited toward your first month's m
